  1. Are you currently awaiting the results of a COVID test?
  2. Have you been notified in the last 14 days, that you were a close contact of a person who tested positive for COVID?
  3. Have your returned from abroad in that last 3 days?
  4. Are you currently suffering from any of the symptoms of COVID such as:
    1. Fever
    2. Altered sense of taste or smell
    3. Dry cough
    4. Shortness of breath
    5. Muscular pains
    6. Diarrohea

I also ask you to please contact Obus if, within 14 days of receiving your treatment, you develop any symptoms of COVID, test positive for COVID or are notified you were a close contact of a person who tested positive for COVID before coming for your treatment.  This is to allow your therapist to be tested.
